The show begins with Mark Briscoe and "The Heavy Metal Rebel" Frankie Kazarian having already brawled their way into the ring, their match already in progress. Caprice Coleman guests on commentary for this match.

"The Heavy Metal Rebel" Frankie Kazarian beats Mark Briscoe following a double-foot kick to the outside on Jay Briscoe - who shows up during the commercial break to watch from ringside - from Kazarian followed by a roll-up after Mark Briscoe misses the Froggy 'Bow. After the match, Jay Briscoe attacks Kazarian, then "Almighty" Christopher Daniels shows up to make this a tag team brawl. Punishment Martinez suddenly shows up and attacks both Kazarian and Daniels as well as Scorpio Sky, who shows up at around the same time and fares no better. Scorpio Sky gets laid out with a Jay Driller while Daniels receives a South of Heaven choke slam.

Jonathan Gresham vs. Jay Lethal in a 30-minute Iron Man Match for the ROH World Championship is tonight!

We see post-match footage of The Dawgs' (Rhett Titus and Will Ferrara) attack on Cheeseburger, where The Dawgs lay Cheeseburger out after losing to him and Ryan Nova.

We see footage from last week where "The Last Real Man" Silas Young confronts Bully Ray for leaving him high and dry in last week's main event. Bully Ray reminds Silas that he was the bully before he showed up, and tells Silas to come see him again when he finds his killer instinct again.

Jonathan Gresham vs. Jay Lethal for the ROH World Championship in a 30-minute Iron Man Match ends with both men having one fall apiece. In the last five minutes of the match, Gresham scores the first fall of the match by getting Lethal to tap to the Octopus, but Lethal ties it up with the second fall of the match soon after by reversing a roll-up into a Figure Four Leg Lock on Gresham's bandaged left leg, in which he has to chop Gresham in order to get him to tap out within the last 90 seconds of the match. After the match, Gresham says there is no way he is letting Lethal leave the building with that kind of decision, and demands five more minutes. Lethal simply says "Start the clock," and five minutes of sudden death action is authorized by referee Todd Sinclair. Jay Lethal retains the ROH World Championship against Jonathan Gresham in sudden death.
